Multiple Methods

Employers who need to verify income withholding orders, report new hires, or update company information should call the dedicated employer line. This is separate from the general public line and staffed by specialists who understand wage garnishment procedures.

Government-issued photo ID (driver's license, passport, or state ID). If you're the custodial parent, bring the child's birth certificate and Social Security cards for all parties.
Recent pay stubs (last 3 months), last year's tax return, W-2 or 1099 forms, and documentation of any other income sources. Self-employed? Bring profit/loss statements.
Any existing child support orders, custody orders, divorce decrees, or paternity judgments. If you don't have copies, the office can often look them up, but bringing them speeds the process.
